Using the mail service pharmacy begins with two easy steps.

1. Obtain a prescription from your provider for each maintenance drug (typically a 90-day supply) that you or a family member will be taking. Additionally, you may wish to ask your provider to write a 30-day prescription to fill at a local pharmacy while waiting for the mail service prescription to arrive.
2. Complete and mail the Mail Service Enrollment Form, along with your new prescription(s) and payment to:

   Or, have your provider fax the prescription(s) and completed order form to 800.905.9815.
   
   That's it. Your prescription will be mailed to you 10 to 14 days from the time the order is placed. You'll receive free standard deliver on your prescription orders, and rush orders can be shipped overnight to the address of your choice for an additional cost.
