Consumer Health Advantage is a PPO plan with separate in- and out-of-network deductible and coinsurance accruals — perfect with strong networks.

Consumer Health Select is a PPO plan with a combined accrual of in- and out-of-network services — best for limited networks.

Consumer Health Freedom is an indemnity plan typically available only to groups in ZIP codes without network physicians or hospitals.
