Lab Card® Program

This program provides outpatient laboratory testing through Quest Diagnostics at no cost* and is included with the following plans:

  • Starmark Signature Series
  • Starmark Healthy Incentives® PPO and Indemnity
  • Starmark PPO Advantage
  • Indemnity and PPO plans with deductibles of $1,000 or less

Lab Card Select Program

This program offers outpatient laboratory testing at significant savings compared with other labs when testing is directed to a participating Quest Diagnostics laboratory. The program is included with the following plans:

  • Starmark Consumer Health Series
  • Starmark Healthy Incentives® CDHP PPO and CDHP Indemnity
  • HSA Select
  • Qualified High Deductible plans
  • Indemnity and PPO plans with deductibles of $1,500 or more
