Lab Card® Program

This program provides outpatient laboratory testing through Quest Diagnostics at no cost* for all Signature Series and PPO Advantage plans, as well as Indemnity and PPO plans with deductibles of $1,000 or less. Visit the Lab Card Program website to locate a collection site.


Lab Card Select Program

This program offers outpatient laboratory testing at significant savings compared with other labs when testing is directed to a participating, Quest Diagnostics laboratory for all Consumer Health Series, HSA Select and Qualified High-Deductible plans, as well as Indemnity and PPO plans with deductibles of $1,500 or more.
