YourCare Health and Wellness Outreach Program

YourCare* is our integrated health and wellness outreach program that promotes wellness and prevention by encouraging face-to-face interactions between covered members and their physicians. Members receive proactive, timely and personalized reminders for screenings and preventive tests, notifications of apparent gaps in care for common chronic conditions, one-on-one clinical coaching for members with numerous medical issues and assistance with hospital discharge and after-hospital care.
